Insurance

For patients with dental insurance, we are happy to work with your carrier to maximize your benefit and directly bill them for reimbursement for your treatment. However, if we do not receive payment from your insurance carrier within 60 days, you will be responsible for payment of your treatment fees and collection of your benefits directly from your insurance carrier.

Dr. Gleckner participates with the following dental insurance providers:

Dental Benefit Providers (UnitedHealthcare) including HealthAllies, Aetna PPO, Metlife, United Concordia PPO, Cigna PPO, DenteMax, Principal, Horizon Dental PPO of New Jersey, Connection Dental (owned by GEHA), which includes Guardian DentalGuard Preferred and Guardian DentalGuard Preferred Select, Delta Dental Premier, Advantage and Delta PPO plans.

Please Note: Each patient is responsible for verifying their own dental benefit coverage and network participation prior to treatment.

Office Policy

Our office requires payment prior to the completion of your treatment, therefore any treatment over the cost of $300.00 will be pre-determined by your insurance company prior to treatment. Upon receipt of your insurance pre-determination, payment options will be discussed prior to your appointment. For treatment under the cost of $300 (excluding hygiene), an estimate will be made, if insurance applies, the estimated amount will be due at the time of service. If no dental insurance applies, then payment is requested in full at the time of service (see payment options above). For patients requiring multiple appointments, alternative payment arrangements may be provided. If a patient chooses to discontinue care before treatment is complete, they will receive a refund less the cost of care received.
